Medical restrictions by way of a completed Occupational Fitness Assessment Form (OFA) will no longer be done via phone.
Effective Wednesday, March 30/22, you will need to attend the Local 500 office in person between the hours of 9-11 AM on any Wednesday.
Note: You cannot just show up on Wednesday with your paperwork. You must submit the completed OFA / medical documentation on the Tuesday before the meeting. Submitting by email or fax is acceptable. The medical info must be filled out and stamped by your doctor. Forms can be accessed on our website at www.ilwu500.org under the Member/Casual Services tab. Look under Benefit & Medical Forms.
