Members and Casuals Seeking Medical Exemptions / Restrictions

You may no longer simply drop off an OFA (Occupational Fitness Assessment Form) at the Local 500 office.  You must attend a joint ILWU/BCMEA meeting.  Until July 7, 2015, the meetings will be on Fridays between the hours of 9 and 11 am only!  As of July 8th, the following will apply:

Effective Wednesday, July 8, 2015, Members/Casuals requesting medical exemption are required to attend a joint (ILWU/BCMEA) meeting in order to have your restriction reviewed.

These meetings will take place:

  • EVERY WEDNESDAY (until further notice).
  • at the ILWU Local 500 Office
  • #100 – 111 Victoria Drive, Vancouver, BC
  • 9 am – 11 am.

You must bring an updated OFA form with you to the meeting. These must be filled out by your Doctor.  You can print one off from down below, or you can print one off our website at www.ilwu500.org under Member/Casual Services. Scroll down ‘til you see OFA (Occupational Fitness Assessment forms).
